The series is based on the webcomic titled Jonbeodoctor by Kim Tae-Poong, which was first published on February 25, 2022, via Kakao. During its early development stages, the project was known by the working title Jonbeodoctor, translating to "Holdout Doctor". The literal Korean title for the television production is Doctor Island Boy, with the revised romanization recorded as Doctor Seomboy.

In the Enameled broadcasting schedule, Doctor On The Edge occupies the Monday and Tuesday 22:00 time slot. It follows the broadcast of The Scarecrow and precedes Dream to You, which began airing on July 13, 2026.

Plot Synopsis

The narrative centers on Do Ji-Ui, played by Lee Jae-Wook, who works as a plastic surgeon at a university hospital. To fulfill his mandatory military service requirement as a public health doctor, he leaves the hospital. Due to a personal trauma related to the sea and islands, he attempts to avoid a remote posting. Despite his efforts, he is assigned to Pyeondong-do, a remote island village frequently avoided by public health physicians.

On the island, he crosses paths with Nurse Yuk Ha-Ri, portrayed by Shin Ye-Eun. Yuk Ha-Ri previously worked at a university hospital before moving to Pyeondong-do, where she maintains dedication to her patients while harboring a personal secret. Through his medical service on Pyeondong-do, Do Ji-Ui interacts with local villagers and navigates his responsibilities.
